How to apply for Medicaid or NCHC
Applications are taken at the local Department of Social Services - Stanly County Commons, 1000 North First Street, Suite 2, Albemarle, during normal hours of operation. Normal hours are Monday through Friday, 8:30am until 5:00pm.

All these items are needed for both spouses.

Purpose: To pay medical expenses for income and reserve eligible aged, blind and disabled persons.
Medicaid
to the Aged, Blind and Disabled (MAABD)
This program is for persons 65 and older or, if under 65 having met the Social Security definition of disability. This is a full Medicaid program that pays for Medicare premiums, hospital bills, doctor's office visits, and prescription drugs. Effective January 1, 2006, Medicaid recipients, who also have Medicare, will receive drug benefits under Medicare. It also helps with the cost of care in a nursing facility. Income and reserve requirements must be met.
If the income exceeds the state guideline, a deductible must be met based on income. A deductible is the amount of medical expenses that must be incurred before Medicaid can be authorized, for someone living in a private living arrangement.
For a Long Term Care situation, a Patient Monthly Liability (PML) is assigned to the nursing facility, based on income. This is the amount that a person owes for their cost of care to stay in the facility.
Medicaid for Qualified Beneficiaries (MQB-Q)
This program is for persons that have Medicare, parts A and/or B that meet income and reserve requirements. This is a limited Medicaid program that pays for Medicare premiums, hospital bills, and doctor's office visits. This program does not pay for prescriptions.
Medicaid for Qualified Beneficiaries (MQB-B,
E)
This program is for persons that have Medicare, parts A and/or B that meet income and reserve requirements. This programs pays for Medicare premiums only.
This program is for persons living in a domiciliary (rest home) care facility. Eligible persons receive a supplemental check to help pay for their stay. In addition, they receive full Medicaid benefits.

How to apply for Family & Children's Medicaid or North Carolina Health Choice
Applications are taken at the local Department of Social Services - Stanly County Commons, 1000 North First Street, Suite 2, Albemarle, during normal hours of operation. Normal hours are Monday through Friday, 8:30am until 5:00pm. An application may also be picked up and taken home to fill out for Family & Children's Medicaid or NCHC for Children. Applications are available at Social Services, at the Health Department, and at the Public Library. The two-page application may be mailed back to the DSS office or returned in person to the office. An interview is completed at DSS. The application must first be evaluated for Medicaid eligibility. Anyone not eligible for Medicaid is then evaluated for NCHC. In order to process the application more quickly, verification of the previous month's income and social security numbers of all applicants and caretakers should be attached to the application. The family will be notified of the approval or denial of benefits as soon as eligibility is determined.
Medicaid
for Pregnant Women (MPW) 
Medical assistance is available for pregnant women who fall below the income guidelines. The coverage is for pregnancy related procedures only. Once determined eligible for MPW, the coverage continues through the postpartum period regardless of income or other changes.
Medical assistance for low-income families with children under 19. To qualify the family must meet income guidelines and resource limits (bank accounts, vehicles, and other assets). This category includes 19 and 20-year-old individuals that may or may not have a child. Some families may have a deductible to meet before Medicaid coverage begins.Medicaid for Infants and Children (MIC)
Medical assistance for children or individuals under the age of 19. There are three different income categories based on the age of the individual. The income limits are highest for very young children. There are no reserve limits or deductibles for this program.
North Carolina Health Choice (NCHC)
Medical insurance administered by Blue Cross Blue Shield. This program is for individuals under age 19 who’s income is too high for Medicaid and who do not have private insurance. Depending on the family’s income there may be co-payments for prescriptions, doctor visits, and emergency room visits. Some families may need to pay an enrollment fee of $100 maximum before benefits are issued. This enrollment fee covers the child/children for one year.

Carolina ACCESS is a managed care program that provides a more efficient and effective healthcare system for Medicaid recipients. This program links Medicaid recipients with a primary care provider who delivers and coordinates their health care needs. This improves patient access to physicians, enhances patient/Doctor relations, and reduces inappropriate or unnecessary use of medical services.
